How to examine a patient using an otoscope?

Examining a patient using an otoscope is an essential step in checking for any ear, nose and throat (ENT) problems. An otoscope is an instrument used to inspect the outer and middle ear and to look for any signs of infection or inflammation. It is important to know how to use an otoscope correctly in order to accurately assess the patient’s condition.

Put the patient in a comfortable position

Before examining a patient using an otoscope, it is important to ensure that the patient is in a comfortable position. Depending on the patient's age and condition, you may want to ask them to sit up or lie down on the examination table. It is also important to allow the patient to hold the otoscope if they would like, as it can help them feel more at ease during the examination. Once the patient is in a comfortable position, you can begin the examination.


Insert the otoscope into the ear canal

After you have sat the patient down in a comfortable position, and thoroughly explained the procedure to them, you can begin the examination. The first step is to gently insert the otoscope into the patient's ear canal. To do this, hold the otoscope at the tip and angle it slightly towards the patient's ear canal. Be sure to move slowly and cautiously, and ask the patient to let you know if they feel any discomfort. Once the otoscope is in the ear canal, activate the light and adjust the focus if necessary.


Adjust the light setting

Before you start examining the patient, it's important to adjust the light setting on the otoscope. This will help you get a better view of the patient's eardrum. To adjust the light setting, look for the small dial or toggle switch at the bottom of the otoscope. Turn this dial or switch to adjust the brightness of the light. If you are using a disposable otoscope, the light set will be automatically adjusted. Once you have adjusted the light setting, you are ready to examine the patient.


Examine the eardrum and surrounding area

After taking the patient's vital signs, the next step is to examine the patient's eardrum and surrounding area with an otoscope. Make sure the patient is looking straight ahead and the ear is facing up. Using the lighted end of the otoscope, carefully look into the ear canal. It may be necessary to pull the earlobe up and back for an adult, and down and back for a child. Look for the eardrum, which should appear grey and translucent. If there is any wax blocking the view, carefully use an ear curette or cotton swab to remove it. Examine the surrounding area for signs of infection, such as redness, drainage, or swelling.


Clean and remove the otoscope

After the otoscope is turned on and ready, it’s essential to clean and remove the otoscope. This should be done by using alcohol wipes or sterile cotton balls, depending on the type of otoscope you are using. When wiping the otoscope, do not touch the lens or other parts of the device, as this could spread germs and/or dirt. Once the otoscope is clean, carefully remove it from the patient's ear and store it in a safe location.
